SURGEON: Dr. Siamak Agha MD, PhD, FACS

3-WEEK FOLLOW-UP TRANSCRIPT:

So I wanted to give you an update on our patient before she goes back to Hong Kong. She's been here for three weeks, healing really well. Yay. And she's been taking a bunch of peptides. So far, so good. We are not out of the woods. Recovery takes about six weeks, but so far, very nice.

Tell us what you had had before you came to us. I'd had an abdominoplasty, which was slightly extended to come around here. Yes. I had also had a BBL done to try and, again, lift the skin through putting fat here, but didn't get the results I wanted. And I'd had the inner thigh lift, and again, that had migrated down through the labia, so down the inner thigh and also at the back as well.

Yeah. So she had her inner thigh lifted, back of thigh cut, BBL to really fill her buttocks up, and a tummy tuck. So what we ended up doing is to convert everything into a high-definition tummy tuck. We obviously didn't have to do her tummy tuck again, but we did a scar revision in the front there. And look at how beautifully she's healing.

I didn't have to cut her inner thighs again, so we lifted the inner thighs through a different procedure that I offer. Sometimes I don't have to cut people. This is the front of her thigh that is elevated, the outer thigh that is elevated. This little hole represents where the drain came. The drain went into her buttocks.

So we put a flap here. The flap will go down. Obviously, right now everything is swollen and bigger, but this flap will go down and will probably end up with this. We put fat into her buttocks, and then I corrected the placement of the posterior thigh.

It's overcorrected so that as the incision drops back and perhaps migrates a tad, it's still going to be covered by the buttock. Meaning that when I do the posterior thigh lift, posterior means back, lifting the back of your thigh, which we did for her, I try to secure that incision and that placement and reconstruct the infragluteal crease, the crease underneath the buttocks.

And this is her prior incision. You see the end of her incision. So I didn't even cut all of this. I tried to keep my incision to underneath her butt, but this is a remnant of the previous work that was done on her on that side and on this side. She scars remarkably well, but the scar comes another inch on each side.

So was it worth coming all the way from the other side of the planet? A thousand percent. And what peptides are you taking? I'm using a mixture, ones which you recommend. BPC-157, GHK-200, and there's a third one, TB-500.

Right. And you believe that it's making a difference, right? I think it makes a huge difference. That, nutrition, following your instructions to the T. Yeah. Your pre-surgery protocol, I think, was really beneficial, all of the nutrients you recommend.

Right. And doing that before and after, I think all of it holistically together makes a big difference. Yeah. Every aspect of this makes a small difference. Once you add it up, it makes a tremendous difference, and it's all important.

And as I tell everyone, recovery is half the surgery. Following instructions is half the surgery, and she's an example of that. Three weeks out, and we are doing really well. Yes. No, I'm super happy. Really happy. Thank you.

Surgery Plan

SURGERY PLAN TRANSCRIPT:

Today we are planning a comprehensive lower body transformation. Our patient has flown kindly from Hong Kong, where she resides. And many years ago, she lost about 50 pounds of weight and ended up having a tummy tuck about 15 years ago in London. In London. Yes. And then she's had a spiral thigh lift, which is a procedure that we described in 2012 that lifts the back of the thigh and the inner thigh, and that was performed in Los Angeles.

And she's now here to do essentially a lower body lift to tackle the issues that remain. She works out every day, literally every day, because that's her job. And despite that, as you can appreciate, we have some elements of skin looseness, especially on the thigh area and the buttock area.

So the plan of surgery today would be to perform a tummy tuck scar revision, pubic lift, and essentially remove her old scar and utilize that incision to lift the front of the thighs, the outer thighs, and extend the incision to perform a buttock lift and buttock reshaping.

So once we've done that, let me rotate her. Once we've done that, then we can excise. You see her old incision. We can excise these incisions, give her a much nicer incision, and use that access to lift her buttocks. Her buttocks are here and really need to be lifted to here.

And then this is the incision for the posterior thigh lift that was performed in LA six years ago. And what you'll notice is that despite the fact that she has the incision, the infragluteal crease, this crease was not reconstructed as a crease, and therefore it's still very loose.

So today we'll do a revision of this and reconstruct that like this, and then lift her buttocks like that and reshape them. So we are going to remove inches and inches of skin, as you can appreciate she has it, and then utilize all the excess fat that we have to shape her buttocks.

And literally, we will be saving every gram of fat that you can imagine. We won't be throwing anything away because we need it. Are you excited? I can't wait. I can't wait either. Okay. Okay, let's get going.

Surgery Result - Back

TRANSCRIPT

So on the back, essentially, I performed a buttock lift. I removed her old tummy tuck incision that was on her outer buttocks and removed about five inches of buttock skin. That skin came from the upper buttock and lower back, and then I utilized the tissue underneath, which was partly from the lower back and partially from the upper buttocks, to create flaps. Those flaps were then used to augment the buttocks. The flaps were actually placed on the buttock muscle as I was lifting the buttock skin and tissue over it. In that manner, we reshaped her buttocks and augmented them, preserving every ounce of fat and utilizing it for her buttock reshaping. Coming to the posterior thighs, posterior means the back of the thighs. She had previously had an incision in the infragluteal crease, but the infragluteal crease was not reconstructed, meaning that they had placed the incision, they had removed skin, but the whole incision was loose and therefore had migrated down. So what we did was excise the incision and the scar with it, remove additional skin, lift everything, and anchor suture it so that we could create infragluteal crease integrity. You see, this doesn't move anymore, and that has to do with the fact that it's been anchored with sutures. Therefore, this will contribute to the buttock shape and roundness because now this incision will not migrate down, creating an elongated buttock. Essentially, we controlled the height of the buttocks and the width of them and revised her spiral thigh lift on the back by meticulously reconstructing her infragluteal crease.

Surgery Result - Front

TRANSCRIPT

So on the front, I performed a tummy tuck scar revision, lowering her tummy tuck incision by about one and a half to two inches. And I redraped the pubic area, given the fact that it had been made too narrow, so we made it more triangular and then completed the outer thigh lift in the front. The inner thigh lift that was previously done looked good, so we didn't actually have to redo that portion of the surgery, but the front of the thighs was lifted in continuation with the outer thighs.
