8-MONTH FOLLOW-UP TRANSCRIPT:

Our beautiful patient is here for follow-up. She lives in Arizona. Eight months ago, I did a lower body lift on her, and as you can appreciate, she's not a big person. She's not somebody who's lost a lot of weight. We also did an inner thigh lift.

So the question is, why would you do this big surgery? I work out a lot. I'm very athletic, and my skin, from just, I think, hereditary factors, was very loose. And to be honest with you, I just couldn't look at myself in the mirror every day and feel that way, knowing that I wasn't happy with all the hard work I put in.

Right. To get to see yourself like that all the time. So it was worth every second of pain and any little things I had to come to the best to get it done because the result is amazing. Thank you.

So we did, obviously, our high-definition tummy tuck. Look at her beautiful abdominal contour, pubic lift, front thigh lift, outer thigh lift, inner thigh lift.

So look at this groin incision, guys. Look. It's there, okay? And I can't show you more than that. But you see how the groin indentation has been perfectly reconstructed, and the pubic area is actually correctly sitting over the groin. That's the way it's meant to be done.

A lot of times I see surgeons suturing the skin of the upper thigh to the mons area, and you lose that beautiful indentation, which you are not meant to. The pubic area then spreads to the upper thigh, and then you get labial spreading. Correctly placed, and that has to do with the technique of doing it correctly.

And let's look at your backside. Did an outer thigh lift, and then we did a buttock lift and buttock augmentation with all the little fat and tissue that we had. We did flaps. So these are her flaps. You can see these were taken from here and rotated in. Yeah. And this is the other side.

And you see, this is the white scar of the incision. The redness is going to fade. She's going to end up with a beautiful white line ultimately, just like her front.

Surgery Plan

SURGERY PLAN TRANSCRIPT:

Happy Thursday, everyone. Today we are doing a lower body lift for this beautiful lady who's from out of state, and she works out. She's extremely fit and lean, but she has skin laxity. Why do you think you have the skin laxity? Probably from a little weight loss, for sure. Not a lot, but some over the years. And I think a lot of sunning.

I did a lot of sunning and a little bit of hereditary factors in my skin, but the sunning, I think, took a lot of my laxity away. Yeah, it makes sense.

So, as you can appreciate, she's here for a complete lower body transformation. She has had a prior tummy tuck, and she has had an inner medial thigh lift that has resulted in labial spreading.

Our aim today would be to revise her tummy tuck and do an inner thigh lift redo. Those are really some of the hardest surgeries that I do because they tend to want to migrate back, given the fact that normally too much skin has been removed.

We'll also be doing a complete and comprehensive thigh lift, butt lift, buttock reshaping, and buttock augmentation with flaps and fat to take care of these issues.

As you can see, her butt has become elongated, and she doesn't have a ton of tissue, but by the time we are done, we're going to take all this tissue and kind of bring it together to create projection and fullness while we are lifting the buttocks and removing all the excess skin.

So inches and inches of skin will be removed in this manner, and then the outer thigh will be lifted. And these procedures have to be done correctly. I have so many patients who come for redos, and essentially it's because they've had an inferior body lift.

Yes, not everybody does the same thing. Yes, there are different ways of doing procedures, and just cutting tissue and suturing it is not enough. You really have to do the full thing, which means elevate, reposition, reanchor, and then close incisions.

And that's how we do it, and that's what I've been doing for the past 20 years. And if you go back and look at my photos or pictures of body lifts that are 10 years old, you can still see the correction that has lasted for many years.

Are you excited? You're ready. Beyond. Are you nervous? I'm not nervous because it's you. I'll be honest. Thank you.

So one of the hardest procedures that I do is inner medial thigh lift redos. These procedures are typically difficult to perform in the first go, let alone a revision.

Our patient has had scar migration and labial spreading. Her groin crease should be here, whereas her labial skin has migrated past the groin crease by about one inch.

To correct this, we have to redo and reconstruct the groin crease meticulously. These are hard procedures because typically too much skin has been removed from the upper inner thigh, and you don't have enough skin to advance towards the groin and reconstruct the groin crease.

But again, it's one of the procedures that we do routinely in our practice.

TRANSCRIPT

So we have already been at this for a few hours. As you may recall, our patient is not a weight loss patient, but wishes to have a lower body transformation due to aging and sun worshiping, essentially. Essentially, she has skin laxity, rolls, and dimples, and wishes to have much tighter buttocks and thighs, as well as abdomen. Her buttocks were pretty important to her, and therefore, to achieve the maximum augmentation, we used her lower back and upper buttock tissue as a flap, meaning that it comes with its own blood supply, and we placed the flaps over the gluteal muscle to augment her buttocks. This is different from a Brazilian butt lift. With a Brazilian butt lift, you tend to lose 25 to 50% of the fat, depending on whether you're a weight loss patient or not. You lose more in weight loss patients and less in non-weight loss patients, in my experience. But you tend to lose some of it, whereas here we shouldn't lose any of this fat because it came as a flap. I also liposuctioned her lower back and waist, created the concavities of these areas, and transferred some of that fat into the lower buttocks. So that fat is prone to resorption to some degree, whereas the flap is not. But a tremendous change. You can see how her previous infragluteal creases are symmetrical, how her buttock width and height are symmetrical, and how beautiful her incision is.

So we are now finished with our surgery, and we ended up performing a tummy tuck revision, during which time I converted her standard tummy tuck to a high-definition tummy tuck. I also performed further abdominal muscle tightening because we wanted the best result for our patient. We continued with lifting the thighs in the front, and then the inner thighs were lifted, and her groin crease was meticulously reconstructed. And this is the final result of our work.
